Sounds like you are making assumptions about last mile logistics. I have gotten plenty of orders that came to me by a 3rd party. While shipping isn’t what I do for DT, I can tell you we have removed the places we know we can’t deliver to.

That’s the mail service of the US government, so the package would likely be delivered by the mail service of your own country’s government.
International government mail usually gets delivered by the destination country’s government mail. If you ship something that way to the US, it would be delivered by USPS for example.

Can confirm (for Norway), Posten AS delivered the implants the last mile here. 
We have no USPS here.
